- Member Data Entry Sheet (pdf) - used by members to provide complete contact information required by the club to complete the entry of members into the club's database. Please complete all sections of the form. All information is maintained on a secure system with access limited to members in good standing. Personal information is not provided to, sold or used for any purposes other than official Rotary business.
- Prospective Member Contact Form - non-members interested in joining a Rotary club are encouraged to complete this online form. Entries are sent to Rotary International's Membership Department who then forwards your information to Rotary club(s) close to the interested party's home or business.
